Ive had this happen... and it was my fault. I failed to submit a required replacement form on a LTC case I wrote many years ago. Ultimately it was a small fine and 6 months of probation (not suspension)... but it is now permanent on my record and I learned my lesson.
Never will forget the sense of doom and gloom sitting in front of those officials at the Dept of Insurance. They had this state trooper kind of demeanor.
But, take heart. Its a simple investigation and they want to hear your side of the story. They will send you documents explaining what its about in due course before you need to appear.
But to my knowledge, there is no simple period, random sweep of agents to "check up" on them. Someone may have filed a complaint against you and you dont even know who or for what reason yet.
