^ I put together a template for our cancellation forms so you just fill in the fields. WAY EASIER that way. Make sure your specify the fields as READ ONLY and it's actually somewhat finicky. For example...you fill out a whole cancellation form & then all of a sudden in takes info from one field & copies it into another. REALLY annoying
