Yes, I am familiar with that form, and it doesn't really fit his situation, so he is appealing, and the next step would be a hearing, if necessary.

See the appeal form referenced in one of the links I gave you above. You have to expand the post to see all of it. This is the appeal form referenced in all SSA letters giving a determination and saying you have the right to appeal.
