Okay, I guess what I want isn't clear. I just want a questionnaire that will get the info I need for a quote and it will ask questions about health history, prescriptions, that will flag ahead of time any rate-ups etc.
Instead of printing this form out, I want to type it in and save it. Just a client info sheet that's all.
When a client fills that out, it generates an email and sends it to me with all of their information. If you go there and fill one out, I'll send the email I get back to you so that you can see what I see.
