Did you contract through an FMO or did you contract directly with the company? Thanks for any input.

They have almost always sent a confirmation. On one occasion I sent 3 apps and received a fax confirmation on my end but they claimed they never got them. When I refaxed the apps with the confirmation they put them at the front of the line. The only issue I have had with them is a few declines due to less than 2 years since an increase in dosage for any high blood pressure or diabetic drugs. They want to see that it is under control before they issue.
