"Beginning with January 2019 effective dates, Horizon Blue Cross Blue Shield of New Jersey will include medical questions on the Medicare Supplement Plan application for individuals ages 65 and older. Applicants will be required to answer a series of questions (called medical underwriting) if they are outside of their Medicare Supplement Open Enrollment Period (OEP) or do not have a Guaranteed Issue Right. Applicants’ answers will determine acceptance into the plan, as well as the rates they are issued. Additionally, rates will be determined by gender, tobacco use, and in some instances, age."
