Colonial requires 15 benefit eligible employees in order to get the GI with no pre x. Employees need to work 20 hours per week to be eligible. Many churches employ part time workers. The group may be able to reach the 15 minimum with a few part timers thrown into the mix.

the gi with no pre-x is for the employee only. for the dependents it is NOT guaranteed issued
 
Ask to speak with an underwriter about it being 12 vs. 15 lives. There's a chance you can get them to waive it.

Aflac has a hospital plan that pays big for the first 5 days then scales down. Also will pay surgery benefits and out patient service along with a wellness benefit. Very affordable
