What I see happening...
Insurance agents are going to have some form (separate from the application) that each client needs to sign that states they can go to any provider that accepts Medicare and agrees to the terms and conditions of the PFFS plan.
How else would Medicare go about this? They have no control over what the agent says in the home.
Secure Horizons has a form something similar to what you are alluding that states this that MUST be included with your bz. or will not get processed.
I also read that verification calls are going to be implemented. I know Humana does this ( and IMHO I think it is good to do at point of sale, b/c it's mostly to cover an agent). They will also be implementing outbound follow up calls to benefiaries (from what I read) to ensure their satisfactory of the product as well as the agent's disclosure of the product.
