Looking for this also, if anyone could post a link it would be greatly appreciated!! 
Searching the web I could only find the old generic SOA's where the explanation for same day is still needed

I found this one, but it doesn't have the CMS approval codes on it, so I'd be hesitant to use this one. Maybe someone will find an updated one.

Technically, the SOA is no longer requires CMS approval. That said, I think I saw a good on from Cigna recently. I'll look and upload when I find it!

It asks to "check the box" for the product you want to discuss. When did this change from requiring initials?

UHC’s is now check the box, noticed that last year during AEP...

On the downside it appears that UHC scope form doesnt say its optional any longer to put prospects address and phone.This adds much more work if you run alot of appointments
