Who will take a 36 year-old male with 300 to 400 blood sugar levels while on metformin. Says he can't tolerate insulin. Good weight. No complications. 2 to 3 years diabetes.

Senior Life will. If there are no diabetic complications (amputation, comas, etc) and the rest of his health is pretty decent he could get the super preferred rates.
 
I may be missing something but wouldn't a blood sugar reading of 300-400 be considered uncontrolled ? Thus being a complication.
 
It would be considered uncontrolled. Usually complication means when your blood sugar remains high for a long time, you could develop foot problems and/or kidney being the most as complications. But diabetes is brutal it creates complications everywhere over time. Also if he refuses metformin and does not want any other diabetic medication, they will usually recommend surgery to reduce stomach size
