Does anybody have a 5 Star and/or Lafayette Life CA final expense application they can post a link or a web site that they can be downloaded from ?

I think the 5 star app is universal for all states (not 100% sure) but I can send it to you. Shoot me an email and I will be happy to get it to you. I don't have Laf life for CA though.
