No, the letter did not come by certified mail. I actually received 2 identical letters. Not sure why they sent 2, but I called and asked and they said to only send 1 back.

Upload? I thought the only 2 options were to send it back by certified mail or by fax, accoroding to the instructions on the bottom of one of the pages.
 
Incredible I've lightly followed this thread for 4-5 yrs . It's exactly why i only deal with the biggest banks like Goldman Sachs,Wells Fargo etc with cd's .Annuitys same thing . I'm 100% convinced if this were ny life or mass mutual you'd never have waited 5 long yrs to get your money back . To sum it all up will people who had $300k or less get all their money back ? What about someone with $500k? That $200k above the $300k limit about what % will they get back?
 
So you are logged into the policy access portal and you click on the documents tab to see your letter. Print off the 2 required forms at the back of your letter and complete the forms. Get the one form notarized. Scan both forms back to yourself and save on your computer. Then back in the policy access portal again -- you click the little button at the top right that looks like a piece of paper to the right of where it says "log out" -- and you will then see 3 tabs on the left of your screen. The bottom tab says "send service forms". From there you can upload the completed forms from your computer....and you'll get a message that says "submission completed" or something to that effect.
 
My letter had no mention of uploading. The instructions are either by certified mail or fax. I would only do either of those two. I think the uploading function is only for service forms such as change of beneficiary and the other forms you can download from the portal. I sent mine back by certified mail with return receipt to make sure they got it.
 
READ your letters carefully and follow the instructions on YOUR letter!

Our letter said the completed, notarized form and the other form could both be uploaded to the portal -- which we did. No problem.

We have verified by phone that the forms were accepted and will be processed starting next week.....with checks being sent out in February....

Just do what the letter says....not what someone on this website says!
