Lincoln Heritage does not even ask that question on their FE whole life to $20,000. Nor do they even ask for a DL #.

On their preffered whole life and term to $100,000, they do ask the question, but, it's not declined for a DUI. They do not ask for the DL# on those applications either.

Forthought and Chesapeake do not ask that question nor require the DL # on their apps. I don't have enough experience with those two companies to know if it's an issue.
