Late enrollment penalty issue

Humana should have sent a reconsideration form to be submitted to the CMS contractor (C2C). Since the LEP is fresh, hopefully this will be an easy turnover as long as client can provide documentation that he did have creditable coverage for the time in question -- which I'm guessing is about 3 years and that client started Part A at Age 65.

Yea its a typical Retire at 68 years old

Apparently, Humana said he send from with no dates, he said he did, he does not have copy

However Humana wants him to get documents from employer and attestation is not enough

I have not seen that before

so working on this

Yeah, Humana shouldn't require proof. Have you reached out to Humana via Vantage? Ask for a copy of what was received. If he did send in a form without dates that would usually provide him with extra time beyond the original due date to submit info unless that expired too.
