Need help with California automobile assigned risk plan

Please help me with CA automobile assigned risk plan forms. God, my insurance teacher never tough me how to fill this form. And I called him, he seems like doesn't know much about it neither. Anyone good at it? May I contact you and get some tips please.

Also, since Caarp only insure for long haul truckers with 750k, if I need help my clients insure another 250k, where and how should I place it?
