My only problem with that was how do I put the replacement form and Hippa form into the PDF.

That's why I faxed them all together I can then turn them into a PDF from my fax

I use pdfcreator. It is free. I use it on my pc to combine the forms I need into one file.

On Ipad you can open a file from dropbox and then open it in another app like Form Tools.

In Thailand we always use iPad or Galaxy Tablet both work pretty good. For me, I use Galaxy Tab 2. 7.0

We use for quote the plan,premium and others information which help prospect make their decision. After they signed in the form we take back to the company to the next process.

I know this thread is a little old but i'm still a little confused. I have Adobe Acrobat on my Android tablet, which allows me to type in the fields and do signatures with the pencil tool. If you use dropbox, how do use save a copy of the completed app without saving over the clean copy?
 
Never used it, but won't it allow you to 'Save as' and give it a new file name?
