If you can get to a kinkos or office depot, you can usually use a computer to log on and get the form.

Or just print one at home, fill it out and mail to the client with a pre addressed and stamped envelope

Same here, i just get all the information and email or mail one to them. Like Hoosier, I normally do not print any in advance. Exemption is if I am going to do service work specifically. Or like Donnie, phone a friend. Just not that big a deal.
