How do they handle proctored exams?

No exams at all.

You have to login and every 30 minutes reply "here" in the chat box. They audit this to make sure you're still in the training.

Most of the training is pretty good, but you could simply put a timer on and check-in every 30 minutes while doing something completely different like making phone calls.

My issue with proctored exams is having to find someone. I typically go another agents office and take it there. I guess you could just do it at your office (without proctor present) but I assume they track the IP address where you take it.
