What Relationship Do You Put for this Beneficiary?

If they are married, they are husband and husband or wife and wife. My married gay clients and friends refer to themselves with those terms.

For what it is worth, I've never liked the term "partner" in this context because it is too ambiguous. "Life partner" makes the relationship clear, but the term is out of vogue.
Some same sex couples still sue the traditional Husband and wife designation. Know a lesbian lady that always refers to her partner as her husband.
 
If they're married I would write husband or wife. If they're not married I would put domestic partner. I would imagine that here in 2021 they couldn't provide much resistance with those answers on the application. No matter what people think about other folk's lifestyle choices all you need is insurable interest. Gay people who live together in a domestic partnership would absolutely be financially burdened if their partner were to pass away so it passes the sniff test for me.
