How would someone dispute information on their medical records. I have a lady who said a doctor diagnosed her incorrectly (he is no longer practicing, at least not in the area).
 
People say that all the time. What was the incorrect diagnosis? Does she have a follow up test that disputes the findings of the initial doc?
 
Have the client call the clinic and ask, they often have a standard procedure form to review the complaint. For example, I had a client denied for health insurance due to drug use.

She told her doctor about five years back that she needed to get out of her lease with her roomate, since her roomate had been smoking a lot of pot. The doctor misheard and put those notes in her records. After going through the formal process, the records were changed, we reapplyed to the same carrier, and she was accepted.

FYI: Be very careful what you tell your doctor. On the rare occasion I'm there, I try to say as little as possible.

No first hand experience, only anecdotal, but I have heard it is easier to clear up credit reports than MIB records.
 
The doctor is no longer in the area. He was new and I guess he diagnosed her with depression. It seems like that would be a hard one to get reversed.
 
To start with the prospect needs to pull their medical records to see what is in them.
 
If you have all the documents from the initial, of all the follow ups then you can proceed even through the doctor no longer continues in the same area.

Yes, but this doesnt help his true medical records.

If there is misinformation on someones medical records it can be a big deal. The treatments for serious conditions can often times be influenced by previous medical conditions.
Plus theres the problem with getting insurance coverage.
