They are required to increase premiums to keep a certain amount of Reserves.

But Carriers request Premium Increases, and those requests are approved by the states.

Premium increases are essentially based on a formula.

And the formula used, is primarily based on the Price of Care.

None of this supports your premise. Not sure what your point is actually.

Premiums on PDP's didn't increase because the costs of prescriptions increased. They increased because the government capped oop costs for beneficiaries.

Notice you didn't include "doctors". And that's the problem in this healthcare debate. It's ASSUMED that because being a doctor is considered a noble profession, that every doctor has noble intentions. It's just the insurance companies that are greedy. But doctors and their professional associations are just as venal as any insurance company. Insurance companies are just playing with bigger numbers.

Hell, in the article I was replying to the doctor implied that anesthesiologist would cut off anesthesia in the middle of surgery if BCBS had implemented CMS work time value values. Even though, any serious "solution" to our healthcare system will involve specialists taking a pay cut anyway.

What evidence do you have that he lied about what their policy was going to be. That has been widely reported and that they pulled back from that policy after significant blow back.
