How do you submit MOO paper app?

Is the MoO <edit> eApp F2F only or can it be done by phone?

I do believe from the way the eApp flows that telesales is permitted. As suranceman said, the client will need an email address in order to eSign. Also, I do believe that telesales will trigger the PHI. I am NOT certain on this, but I seem to remember having read or heard that in some of the training material MOO had put out. I may very well have mistaken that with another carrier, however.
