Not sure if we are talking about the same thing. However, I use a reply card in client and orphan mailing. We get a pretty good response. I have check mark boxes next to questions like "Are there any guarantees?" And "Are my beneficiaries worded correctly?" I have changed it around a bit if I am mailing to people that have a different agent.

No. these are cards people fill out for initial warrantees after purchasing a TV or other appliances. They check off if they are interested in getting more information on life insurance, annuities etc.

What do they charge for these warranty survey cards for insurance?

no charge if you contract with national annuity and life sales
