Is a Medicare Agent affected by Vector the same way a Life Insurance Agent is affected by Vector?

Ok. What about a debt to an agency vs directly to an insurance company? Are both viewed the same?

Yes. If you have a Vector you have insurance debt that was not paid. If it is to an Agency, that typically shows you didn't pay the carrier so the balance rolled up to the Agency and they put you on it. Whether you owe Todd King or Foresters and whether it was a life or medicare supplement policy means nothing....you did not pay a debt incurred through insurance sales and that is frowned upon greatly.

I'm kind of in the same situation. I sold FE years ago and got out of the game but am just recently trying to get back in.

When checking for any vector hits I just got a notice saying I have a debt with Americo.

Once I pay the debt off - will I still have issues getting appointed?

You can get appointed with Senior Life if you have a vector, subject to your upline's approval. Usually depends on if you're being responsible and making payments on your debt.
