American General Life & Acc Surrender Form Needed

Sorry I posted this in the wrong forum earlier. I have a client who is surrendering an interest sensitive WL policy with AGLA in Nashville. The surrender form I have for them indicates that we need to use a form 5004 for this particular policy. Anyone familiar with these or that form?

Jacob: I emailed you the 5004 and a cash surrender form. I think you only need the 5004 if the Master Life plan has the single premium rider attached.

thanks Louis, he did have a single pay rider attached to it. His old agent had converted him from a increasing cost UL to this product and rolled the CV into the single pay rider.
