Same here. This was back in the 80s things were different. It also happened right after I left JH so it cost me nothing. If I mail a policy, at a minimum, I priority with delivery confirmation and all goes in the file.

On the case notes. I have had my notes subpoenaed. It is all about CYA.

Well we have to use replacement forms in IN. And I did. Also signed delivery receipt. Do I really need anything else? I left a vm with my contact at Oxford. He hasn't returned my call, though I think he's on vacation till the first.

According to Newby your state does not include on the replacement for the reason for the replacement. I always look at the replacement form as my own CYA form I put in a mini paragraph on the 1 1/2 lines given for the reason because my notes are not signed by the client and submitted with the application but the replacement form is. I have also been known to submit a letter of instruction signed by the client with further details so no one can claim I am making things up after the fact , granted these are more annuity replacements.
