Look at AmMem and Monu Standard Day 1 100% coverage options.

Monu may not work due to stent plus heart attack; check the application.

AmMem won't due to the heart attack. AmCon will do graded up to 35k. Pretty penny for a smoker, but it is 40/75/100.
 
Is not a heart attack within a year Standard rates? Or is that one of the few that has a 2 year look back?

Heart attack is two years, period. Stents without a heart attack is Standard for first day coverage.

Assurity will do 1 year from a heart attack, but stents are 5 years.

Any graded plans that will go to 50k? Budget is $200 a month.

Sure, 12 months after he stops smoking. I am assuming the T to indicate to tobacco use.
 
To get 50K, you might have to split it up. For example Columbian and Gerber will each issue 25K GI at that age.
