While I'm thinking of it here is a tip when using Form Tools to make applications into forms. Enlarge the page and it's much easier to create the form fields especially the little check boxes.

Looks like a good tool. Do you download this to a PC, build the form then download that to the tablet?

I didn't. I installed the Form Tool on my Ipad and turned the ap's into forms on it. Had to email the ap and open it on my ipad. After you install Form Tool you have the option to open the attachment with it. Then you just turn it into a form. A little tedious. Might be an easier way but I don't know it.
