That's odd. I know of 3 agents who've had 2 DUI's, and kept Insurance and Securities license since it was only a Misdemeanor, and not a felony

Usually once the court settles everything your lawyer or yourself should receive something in writing. I believe this is a court document explaining the matter and how it was resolved.

This shows the insurance comaony that what you told them matches up to the court documents.

Just a random thought, but if the state thinks it's OK for you to have a license, why would a life company feel the need to scrutinize it further?
