Something to point out with 5star, their modal factor for monthly is one of the lowest I have seen (uhl being the highest). Their rate per 1,000 might be more but sometimes the quotes come out cheaper because of the modal factor!

Here in Cleveland, companies to have are:
Monumental: easy app, no pos, awesome for copd
RNA: cheap price, good for replacement
Settlers: uw is good, rates are competitive, they do direct bill
5star: debit cards for SS, no rx but mib, got pos for instant uw decision, or send it in for uw processing and get paid 2 weeks later.
