I'll go back to what I said earlier in this thread. Have the wife call the insurance company direct and request the form....or get it from the website. Then you have no idea when or if she did it and how she got the husband's signature. Simple enough.

Yep -if she wants to do this on her own - its her decision and not an E&O suit pending or insurance commission complaint on file.
