I doubt it is the app wasn't completed correctly, Foresters would have emailed about that explicitly.

Could be appointment or over their issue limit for his age.

That too. Most companies are pretty good about NIGO. I'm going with licensing/appointment issues or over issue limit. And of those, I'm going with over issue limit. The few times I had an appointment issue, if the company couldn't JIT the appointment, they returned the application as well. As if I never submitted it. Of course we won't talk about the electronic copy they kept in their records...
 
It was only for about $3,500. $55/month.

I've checked on the portal and there's no notes. Just says decline.
His letter states that premium was never received and yes, they have been closed since Thursday.
