Directmail Card with Face Amount Options

All of the lead pieces seem to either talk about benefits for pennies a day or mention life insurance etc etc but I recently saw what I thought to be a mutual of omaha directmail piece that included an option for them to check a requested face amount that were in order of I believe 5,000 10,000 and 20,000.. Does anyone know where to purchase these

Any FE Lead vendor in the US will have a version of this. That doesn't mean your returns will be worth using it though.

Settlers has one that I've used in the past. I averaged about a 0.7% return. No better than any other lead card.

Yeah I don't know. It was a couple years ago. Just pointed out that the lead was no better than any other lead card and half the response rate.

Like everyone said, every mail house has this card. But the new and improved version is more exclusive. They took the amount check boxes off the card and added these:

Please have an agent contact me who:

___ Can get me approved for life insurance today
___ Allows me to pay by autodraft from my bank
___ Can meet with me on ___/___/___ at ____:_____ am pm
___ Likes a good home cooked meal
___ Would like to meet my three lovely daughters
___ Hah! You thought this was real at first didn't you?
___ There is no perfect lead.
