Here is a useful HHS.gov website that shows the lowest price Metal on-marketplace premiums in each state. You click on the state and enter the age of the health insurance applicant to see the results.

WHO IS RESPONSIBLE FOR LETTING A MARKETPLACE PURCHASER KNOW HOW MUCH HE/SHE PAID IN PREMIUMS, AND/OR RECEIVED IN SUBSIDIES EACH YEAR?

"The Marketplace will send you an information statement showing the amount of your premiums and advance credit payments by January 31 of the year following the year of coverage. For example, you will receive the 2014 information statement by Jan. 31, 2015, and can use this information to compute your premium tax credit on your 2014 tax return and to reconcile the advance credit payments made on your behalf with the amount of the actual premium tax credit."

MARKETPLACE DEMONSTRATION VIDEO

Here's a good video from the CMS Marketplace Training and Resources website. It demonstrates how to complete the Subsidy Application and Choose a Plan at HealthCare.gov. (Select the "Marketplace Application and Enrollment" video and watch full-screen.)

Charts for all States: Medicaid and CHIP Eligibility Levels | Medicaid.gov

Especially useful for those in state with expanded Medicaid for 2014. No need to waste time getting routed through healthcare.gov, if the applicant is willing to enroll in Medicaid. He/she can go straight to the state's Medicaid website.

(Note: A Medicaid-eligible individual CAN purchase an individual health insurance plan, but is NOT eligible to receive an Advance Premium Tax Credit Subsidy.)
