I usually sell plan G but looking at plan N. The question I have on a plan N I understand you have up to a $20 copay if you go to the Dr. but if the dr doesn't accept assignment then it's 15% but no $20 dollar copay is that correct? (I know it's rare)

Also what is the worst case someone has seen in an out patient surgery where they don't accept assignment? I'm just afraid those excess charges can be expensive.
 
The formula applies regardless of whether doc takes assignment or no.

Plan N does not cover xs charges so that would be absorbed by the patient.

Yes, it can get expensive.
