Yep. A good friend of mine is a drug rep. He use to be in NYC before coming back home. He told me, a lot of the doctors wouldn't speak to him if he called them doctor. They wanted to be addressed by their first name. Some would kick him out of the office and he'd have to come back another time.

My experience is anyone who insists on being called Mr. or Ms. or Dr. outside of their office/clinic/hospital is pretentious. The ego being stroked is theirs, they need it to make themselves feel better.
 
I don't have any issues with it. I'm turning 40 in November and with Seniors I always address them by Mr or Mrs.
